Monochrome Health allows you to extend your health, dental, and vision coverage to eligible family members. Review the categories below to understand who qualifies as an eligible dependent and what documentation is required.
Your legally recognized spouse under federal or state law. Supporting documentation (such as a marriage certificate) may be requested during enrollment.
An unmarried partner (opposite or same-sex) with whom you share a committed domestic relationship and meet Monochrome Health’s domestic partnership eligibility criteria. A completed Declaration of Domestic Partnership form is required.
Your biological children, stepchildren, legally adopted children, or children under legal guardianship up to age 26, regardless of student, marital, or residential status. Disabled dependents of any age may also remain eligible with medical certification.

Final day to update your Flexible Spending Account (FSA) or Health Savings Account (HSA) contribution amounts for the current calendar year.

December 31, 2026
Last day to submit claims and receipts for eligible expenses incurred during the current plan year to utilize your FSA funds before the rollover limit applies.
