Preventive Care (100% Covered — Deductible Waived)
Preventive care is fully covered to help you maintain optimal oral health. This includes routine examinations and professional cleanings twice per calendar year, diagnostic bitewing and full-mouth X-rays, and essential pediatric preventive treatments such as fluoride applications and space maintainers for dependent children.
Basic Services (80% Covered — After $50 Deductible)
When routine repair work is needed, the plan covers 80% of eligible costs after you meet your individual $50 annual deductible. Basic services cover standard amalgam and composite tooth-colored fillings, simple tooth extractions, minor oral surgeries, and deep periodontal cleanings or maintenance.
Major Services (50% Covered — After $50 Deductible)
For more complex dental procedures, the plan pays 50% after your $50 annual deductible is met. Major services include porcelain, ceramic, or metallic crowns, bridges and repairs, endodontic treatments such as root canals, and prosthetic devices like full or partial dentures and relining services.
Orthodontia (50% Covered — $1,500 Lifetime Max per Child)
Orthodontic treatment is covered at 50% for dependent children under age 19, up to a $1,500 lifetime maximum benefit per child. Coverage applies to comprehensive orthodontic evaluations, traditional metal braces, and clear aligner therapies.
Annual Eye Exams ($10 Copay)
You are eligible for a comprehensive eye examination every 12 months for just a $10 copay. These regular check-ups update your prescription, evaluate visual acuity, and screen for early indicators of glaucoma, diabetes, and other health conditions.
Prescription Lenses ($25 Copay)
Standard single-vision, lined bifocal, or lined trifocal plastic lenses are available for a flat $25 copay. Essential lens enhancements—including UV protection, scratch resistance, anti-reflective coatings, and blue-light blocking filters—are covered at 100% with no additional out-of-pocket upgrade fees.
Frame Allowance ($175 Credit Every Year)
Choose brand-new designer frames every calendar year. You receive a $175 retail allowance toward any frame of your choice, and VSP automatically applies an extra 20% discount to any remaining balance above your credit.
Contact Lens Alternative ($150 Allowance)
If you prefer contacts over traditional eyeglasses, you can apply a $150 annual credit toward your contact lens materials and fitting exam. Please note that choosing the contact lens allowance replaces your frame and lens benefit for that plan year.
